ConnectCare is a non profit Medicare certified homecare and hospice agency established through a joint initiative of Hutchinson Area Health Care and Glencoe Regional Health Services. ConnectCare also owns and operates Birchwood: The Mary Burich House. Birchwood is an 8 bed residential hospice home specializing in end of life cares.
ConnectCare serves all of Mcleod County, the South Eastern portion of Meeker County, the Southern portion of Wright County, the Western part of Carver County, the Northern portion of Sibley County, and the North Western portion of Renville County. All requests are handled on a case-by-case basis to ensure that quality standards of staffing and on-call coverage are available.
We accept a variety of payer sources, such as Medicare, Medical Assistance, private insurance, HMO’s, long term care insurance and private pay. Our staff will assist with insurance questions regarding individual cases.
